Requirements for Passports
- Certified copy of Birth Certificate or Old Passport or Naturalization Papers
- One 2"x 2" passport photograph (CVS, or Walmart)
- Photo identification
- Check or money order
- Be sure to use BLACK INK to fill out the application.
Application Fees
MUST BE CHECK OR MONEY ORDER PAYABLE TO "US DEPT OF STATE"

| Card: Adults - $30 | Children under 16 - $15 |
CLERK OF COURTS FEE - $35 (Can be payable by cash, check, money order, or credit card)
ALLOW 9 WEEKS TO RECEIVE PASSPORT
EXPEDITED PASSPORT ALLOW 5 WEEKS
EXPEDITE FEE - $60 + Overnight Postage $21.36
MUST BE CHECK OR MONEY ORDER PAYABLE TO "US DEPT OF STATE"

Total Fee to Clerk of Courts $35.00 + $30.45 = $65.45
Special Requirements for Children 15 Years and Younger
Both parents and child must be present at the time of application.
Adult passports and cards are good for 10 years.
Child 15 and younger passports and cards are good for 5 years.
